How to Register
Catchment Area
To register with our practice please download, print and complete the relevant form below:
- Adult Registration form (aged 16 years and over only)
- Child Registration form (aged 15 years and under only)
Once you have completed the form you will need to return it to the practice and complete a GMS1 registration form (GMS1 Form) as well as provide two documents as proof of identity to complete your registration.
Please email the completed Registration Form, GMS 1 form and proof of identity to chelsea.reception@nhs.net

What identification (ID) do we need?
Acceptable Identification so that we can check the details on your form are accurate; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
Until you have submitted your GMS1 form and had your ID checked you will not be registered at the practice.
Temporary Patient Registrations
If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.
To register as a temporary resident you will need to complete a GMS3 form. Please return the completed form to the surgery.
You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.

Find Your NHS Number
Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.
This online tool will help you find your number if you do not know it.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.
Other ways to get your NHS number
If you cannot get your NHS number online you can:
- find it on any letter from the NHS like a prescription or appointment letter
- call your GP surgery and ask them for your number
